Description

Nationally accredited senior center seeks candidate for the long-term care Ombudsman program! Visit nursing facilities (NF), personal care homes (PCH), domiciliary care homes (DCH) and older adult daily living centers (OADLC) in NW and NE Phila serving as a liaison between the facilities and the residents and their families to facilitate the best possible quality of care for the residents.
Provides information and referrals to residents living in the assigned long-term care facility about their rights, community services available to them, laws, regulations and standards that govern long-term care facilities, and good health and safety practice.
Acts on residents’ behalf in response to grievances and complaints of actions, inactions or decisions on the part of facilities or state agencies that adversely impact residents’ health, safety, welfare or rights.
Determines the facts of an alleged violation, makes a determination that the complaint was/was not founded, works with the resident and service provider to resolve the complaint, and, if necessary, refers the complaint to the appropriate regulatory agency.
Maintains records of complaints and problems received, actions taken, and findings and results.
Encourages and assists residents living in the assigned long-term care facility and their families and friends to become more actively involved in their care and treatment by assisting to develop and participating in resident councils and family support groups.
Represent the interests of residents before government agencies and seek administrative, legal, and other remedies to protect the health, safety, welfare, and rights of the residents.
Attend exit interviews in long-term care facilities coordinates the participation of Volunteer Ombudsmen members.
Prepares statistical reports as required and assists with preparation of other reports.
Along with the Supervisor, provides information about the LTC Ombudsman Program through presentations and community outreach.
Maintains records in accordance with applicable rules, regulations, policies, and procedures, including protecting the identity, confidentiality, and privacy of clients and/or their representatives.
Provides on-call emergency intake coverage periodically as needed.
Performs such other related duties as may be assigned.
Job Type: Full-time
Salary: $36,000.00 - $39,000.00 per year
