Job Summary

This role provides supervision and oversees the case management operations of the Violence Intervention Program (VIP). This position is also responsible for VIP outreach.

Job Responsibilities (Continued)

•    Oversee the day to day case management Operations of the Violence Intervention Program including, intake reviews, documentation, and case management goals
•    Oversee case management assignments
•    Oversee and as needed, perform psychosocial assessments, therapy, and group interventions with clients and/or caregivers
•    Share information regarding patient/caregiver situations with stakeholders including with staff from community agencies and programs
•    Ensure compliance and incorporate standards and regulatory requirements into care
•    Communicate program status to stakeholders and escalate and resolve potential issues
•    Support the hiring and training, of new team members
•    Supervise and develop staff including Case Managers, Community Health Workers and interns

•    Review data bases regularly and discuss barriers and solutions to meet goals
•    Develop and utilize staffing strategies to meet client needs and the client care populations
•    Evaluate staffing models to meet care needs and ensure work/case load distribution

•    Develop and maintain relationships with patients, families, staff, leaders and physicians
•    Promote and ensure sensitivity to cultural diversity through work and with teams.
•    Identify barriers for families in accessing community services and advocate on their behalf.
•    Identify resources to support client and families recovery goals

  •  Other duties as needed.

Program Outreach 
•    Manage referrals and eligibility from various referral sources
•    Conduct initial outreach to youth and caregiver to assess for needs and schedule intake
•    Complete consistent documentation in appropriate databases
•    Coordinate with Research Team regrading eligibility and study protocol

Required Licenses, Certifications, Registrations


Required Education and Experience

Required Education: Master’s degree


Required Experience:

  • Five (5) years of social work or healthcare related experience
  • One (1) year of managerial or supervisory experience

Preferred Education, Experience & Cert/Lic

Preferred Education: Masters degree in Social Work


Preferred Experience:

  • Three (3) years of managerial or supervisory experience
  • Previous experience working with children and diverse populations.
  • Previous experience providing culturally sensitive counseling services to urban minority populations.

  • Basic proficiency with data management systems.
  • Intermediate knowledge of trauma-informed care.
