Position Summary:

As a member of the Interdisciplinary Team, the Certified Nurse Assistant is responsible for assisting with direct patient care as well as other work which supports the patient environment. The CNA assists staff to ensure optimal patient care and assists the healthcare team to provide and maintain a clean, safe, and attractive environment for patients.  Services are provided in a professional, compassionate manner to relieve social and emotional pain of the patient by providing care which restores dignity and enhances self-image.  By giving services to the family via respite, anxiety and exhaustion of caregiver is alleviated/prevented.

Job Responsibilities:

  • Provides directly or assists with bathing, oral hygiene, skin care, and hair care of the patient, changes bed linen.
  • Assists with patient's ambulation and with bed to chair transfer.
  • Turns patients in bed to position of comfort.
  • Assists with pre-poured medications, which are ordinarily self-administered.
  • Prepares light meals for patient and assists with eating.
  • Empties foley catheter drainage bags and records amount.
  • Assists with prescribed exercises which patients and home health aides have been taught by appropriate health personnel.
  • Attends weekly IDT meetings and participates in formulation of the plan of care.
  • Provides care only as outlined in the plan of care unless instructed by the Hospice Manager or designee.
  • Reports patient/family changes as they occur to appropriate Hospice IPU RN.
  • Documents care provided to patient/family in medical record as determined by policy.
  • Must be certified as a CNA in the Commonwealth of Pennsylvania
  • Must have current CPR certification and maintain valid PPD.
  • Must have an automobile, a valid driver’s license and vehicle insurance.
  • Must exhibit ease in caring for dying patients and grieving families.

Knowledge and Experience:

  • Must be certified as a CNA in the Commonwealth of Pennsylvania.
  • Must have current CPR certification and maintain valid PPD.
  • Must have an automobile, valid driver's license and vehicle insurance.
  • Must exhibit ease in caring for dying patients and grieving families.
  • Must complete an orientation/training program re: Hospice philosophy and method of care.

Education:

Must have High School Diploma or GED equivalent.
