You are invited to join the VNA Team as an occupational therapist. As a rehabilitation clinician, you will provide hands-on care, education, and training to patients as they recover from health conditions and injuries. In collaboration with the interdisciplinary care team, you will support your patients’ ability to remain safe at home. Essential Functions:

  • Manage a caseload, giving consideration to patient/family needs, travel efficiency, and service priorities.
  • Plan, implement, and evaluate individual occupational therapy programs and diagnostic tests in conjunction with the prescription and/or the direction of a physician.
  • Conduct treatment planning in consultation with the physician and interdisciplinary team (IDT) to achieve goals for clients and their families.

Other Duties:

  • Instruct patients and family members in supportive, preventive, and therapeutic procedures.
  • Provide consultation to other Agency staff concerning the application of professional principles and procedures to problems in patient care.
  • Consult with IDT members.
  • Identify barriers to desired outcomes.
  • Evaluate equipment needs to increase functional level.
  • Be accountable to the Director of Rehabilitation for the attainment of standards of professional practice.
  • Complete records and reports necessary for patient service and administrative requirements as determined by the Agency.
  • Assume responsibility for professional growth and maintain clinical competence.
  • Establish and maintain effective working relationships with patients, families, peers, supervisors, and community agencies.
  • Be responsible for meeting performance standards, including qualitative and quantitative standards, as stated by the Agency.
  • Identify the need for services of other health care disciplines or Agencies and initiate the appropriate referral.
  • Communicate regularly with the Patient Care Manager or other healthcare personnel to develop coordinated plans for patient care.
  • Supervise the Home Health Aide in carrying out the established plan for care and evaluate the effectiveness of the service.
  • Participate, as appropriate, in the implementation of the Agency's Quality Improvement Program.
  • Participate, as appropriate, in Agency educational activities including new employee orientation, professional student clinical training, and Agency staff education programs.
  • Interpret Agency services and programs to individuals/families and personnel or other agencies.
  • Participate as assigned in community programs.
  • Carry out other activities relevant to therapy services.
  • Comply with the Agency's Policies and Procedures. (Contract therapist also complies with contractual arrangement.)
  • Perform other related duties as requested.

Qualifications:

  • Bachelor's degree in Health Science or related
  • Licensure in Pennsylvania as an Occupational Therapist (OT)
