You are invited to join the VNA Philadelphia team to provide professional skilled nursing services during on-call hours. In this role, you will respond to phone calls from patients, families, caregivers, referral sources, and the community. The ideal candidate is a Registered Nurse with excellent telephone assessment skills, with an ability to solve problems, provide symptom management, and manage concerns as well as service requests. Position Summary: Provides primary RN responsive care to patients during assigned non-business hours. Completes field visits to patients to deliver necessary care and completes all required documentation and communications related to the actions taken and issues addressed.

Essential Functions:

  1. Maintains availability and ability to complete patient home visits as assigned.
  2. Provides necessary care to address immediate patient care issue(s) to stabilize or help relieve patient symptoms.

Other Duties:

  1. Responds to patients/families to address care needs and symptom management issues.
  2. Completes hospice new patient introductory visits and patient evaluations (in hospital, nursing facilities, or at home during on-call/non-business hours.
  3. Makes home visits to patients discharged late in the day from a hospital, nursing facility, or the VNA hospice inpatient.
  4. Performs time of death pronouncements.
  5. Completes records and reports necessary for patient service and administrative requirements as determined by the Agency.
  6. Interprets VNA hospice services and programs to patients, families, referral sources and the community.
  7. Establishes and maintains effective working relationships with patients, families, peers, managers, and Agency external customers.
  8. Demonstrates knowledge of professional nursing principles, practices and techniques and the ability to apply to hospice nursing practice.
  9. Assumes responsibility for professional growth and maintains clinical competence.
  10. Participates, as appropriate, in the Agency's educational activities including new employee orientation and Agency staff education programs.
  11. Complies with Agency's personnel policies and procedures.
  12. Performs other related duties as requested.

Knowledge and Experience:

  1. Current, valid Pennsylvania RN license
  2. Specific clinical experience with Hospice and Palliative care.
  3. Current driver's license and consistent access to a reliable motor vehicle.
  4. Knowledge of hospice admission criteria.
  5. Demonstrates a caring and compassionate demeanor with patients, families, and caregivers.
  6. Demonstrates excellent customer service skills.
  7. Outstanding critical thinking skills.
  8. Strong computer skills with proficiency with EMR system.

Education:

Graduate of an accredited school of nursing with a Registered Nurse diploma, associate degree in nursing, or a Bachelor of Science in Nursing (BSN).
