You are invited to join the VNA team as a RN Care Transition Liaison. This position requires on site presence at a Philadelphia acute care facility (hospital). In this role, you are responsible for coordination of the hospital to hospice transition of care. Responsibilities include conducting the patient/family meeting to discuss hospice services, patient evaluation for hospice eligibility, obtaining consent for services, and coordination of hospice medication, supplies, and equipment orders. We are seeking a RN with prior hospice experience with a caring and compassionate demeanor to provide support and guidance for end-of-life patients and their families. Position Description: The Care Transition Liaison is responsible for developing and managing relationships with hospitals, health care facilities, physician practices, and other community referral sources to generate patient referrals and help expand the AGENCY’s home health, hospice, and palliative care market share.

Essential Functions:

  • Utilizes sales strategies, techniques, and promotional materials to effectively sell AGENCY to accounts in the territory. Establishes and maintains lines of communication between AGENCY and referral sources.
  • Establishes and maintains effective working relationships in assigned facilities to manage the transfer of patients from facility to AGENCY services.

Other Duties and Responsibilities:

  • Generates referrals for home health, hospice, and palliative care by building effective relationships with referral sources across all referral segments.
  • Participates in development of a marketing plan to generate budgeted number of referrals/admissions in the assigned territory.
  • Participates in and contributes to all department sales meetings and sales/operations growth initiatives.
  • Conducts market analysis; develops sales strategy, goals, and plans.
  • Maintains monthly referral report demonstrating activity and results.
  • Works effectively with other members of the territory team and program specialists to effectively grow and maintain the business in the territory.
  • Educates patients, families and facility personnel on the benefits and applications of Agency services.
  • Provides mentoring to new Care Transition Liaisons.
  • Provides liaison coverage at hospitals and facilities as needed.
  • Performs other duties as assigned.

Knowledge & Experience:

  • 1-2 years professional experience in marketing/sales.
  • Knowledge of and experience with community program contacts, resources for care, and education.
  • Knowledge of home health, hospice, and palliative care industries to successfully promote AGENCY services.
  • Demonstrate ambition and drive to achieve sales and marketing goals.
  • Excellent follow up skills with current and potential referral sources.
  • Ability to change the approach in response to different personalities or circumstances/situations.
  • Excellent listening, communication, and interpersonal skills.
  • Valid driver's license, with consistent access to a reliable vehicle with automobile insurance.
  • Ability to adapt to a wide variety of environments.

Education:

Bachelor’s degree in business and/or marketing. Nursing degree is a plus. Graduate of an accredited Registered Nurse diploma or associate degree in nursing may be required for acute facility primary assignment.
